Saw an email today that said LLNL was also going to have a survey. Yes, you will have to enter your PAC and OUN to take the survey. It was presented as a graduate study project.
Anonymous controlled surveys are the best. If you have to identify yourself, you will be inhibited and will you will engage in an exercise of futility!
I for one will not respond to a non-anonymous survey. Taking away the anonymity will deter the majority of folks who might respond and produce survey results that are skewed toward the desired results.
