Los Alamos National Laboratory just issued the following memo to employees regarding an incident that occurred today at the Los Alamos Neutron Science Center (LANSCE):
To employees regarding injuries at LANSCE
This is an informational memorandum from the Los Alamos National Laboratory Communications and Public Affairs Office:
On Sunday (May 3), during preventive maintenance operations at the Los Alamos Neutron Science Center (LANSCE), a Laboratory employee was burned while working at an electrical substation. The employee was transported offsite for medical treatment and is listed in critical condition.
Eight other employees were also transported to Los Alamos Medical Center in connection with the accident; seven of those employees were later released, and one employee remains at LAMC for observation.
The cause of the accident is under investigation.
Due to ongoing activities to ensure the continuing safety of the workforce and facilities, numerous areas at LANSCE are without power and will remain without power until further notice.

Work was being conducted over the weekend. There were three identical looking ~ 12 kV substation electrical boxes. Worker opened the wrong box to clean which was "live". He sprayed conductive "cleaner", electrical discharge, worker's clothes caught on fire. Another worker (a woman) was able to extinguish fire on him. Worker air lifted to Albuquerque with major burns to face and upper torso. Some issues related to whether the worker was an "unqualified" electrical worker or "lineman".
Anyone care to elaborate or clarify? This is what were told out at TA-55.

The other labs in the DOE complex are getting much more information about what happened at LANCE. Funny that we are getting so little information. LANL is a truly strange place.
What happened to lock-out tag-out? Really, LANS? Lans is more concerned with wearing shoes that grip than implementing a lock-out tag-out procedure on high-voltage equipment.

Improperly or untrained workers working for LANS. Workers action are ultimately the responsibility of the company (LANS) they work for. LANS will settle this out of court, unwilling to expose their negligence.

False, LANS does not own this. They cannot control everyone and are not expected to control everyone. Until all the facts come out we should speculate or monger.
