LANL never changes. Lazy, unaccountable operations team. If you are there long enough, you will get the speech on how they invented the process so they dont need to follow the regulations.

Sure the first and second shutdown of Ta-55 for not implementing and then not properly implementing Safety Basis systems and the CSE program.

Let me flip this. Name one LANL nuclear facility that has NOT been shutdown in the past 10 years for an extensive period due to non-compliance with procedures or safety system failures.
