I'm not at Y-12, but without first-hand knowledge of the circumstance, it's hard to know what to make of this. One the one hand, it could indicate sloppiness. On the other, the DOE system has become so deranged that the routine bric-a-brac of process life is now treated like a reactor meltdown. You can't actually run a process of any size of complexity without things going astray here and there. The mark of a well run operation is that these precursors get caught and don't progress close to a significant event.
Sometimes it really is culture. Sometimes it's just stuff happening because minor stuff happens. The one thing I am sure of is that DOE is no longer capable of discerning the difference.

That's not Charlie, it's the Mad Magazine "What, me worry?" kid. If you don't believe me, hold up an old Mad Magazine cover with the kid on it next to that official picture of Ernie Moniz and compare. Same simpering grin and all!
I just want to point out that it is NNSA which is running the labs. I agree that DOE is dysfunctional, but they are beginners compared to NNSA in terms of sheer incompetence. When NNSA got created, DOE gave them the most incompetent idiots, so that their average competence level would go up.
INTERNATIONAL CONCEPT OF WORK FROM HOME Work from home theory is fast gaining popularity because of the freedom and flexibility that comes with it. This poster must not get off the mesa much, otherwise they would know that everyone outside of LANL has been talking about culture again for the past two years. Things did improve for several years after the transition, however they are now firmly back to where they were in the final period of UC.

Yep, our culture is bombs and warheads -- big ones! And we are keepin on "keepin on" with that job. Despite the carpetbaggers stealing some of them to give to a disfunctional lab out West to keep them afloat. You wusses that don't do bombs and other sh--t that blows up need to STFU until you understand what the real, and only, mission of LASL is - bombs and warheads! (unlike LLNL, whose only mission is to rip off the taxpayer with scams like x-ray lasers and NIF).
LANL became irrelevant in the mid-90's when the WWG meetings turned into "here's a new analysis of data we took three years ago. Now it matches the code results!"

Unfortunately, the DOE is unable to discern anything.
Really really old Nuns strolling into the most secure areas of our nation ... that is not "minor stuff."

I would agree that DOE is so mired in rules it can't integrate or think through that it's ineffectual. It just spasms from crisis to crisis.
